Evolve Treatment Center is a premier location supporting adolescents ages 12-17 struggling with mental health, addiction, and/or behavioral issues. Our outpatient programs are geared towards teens whose symptoms and behaviors are disrupting their day-to-day home living. Teens in our Partial Hospitalization Program participate in our full day of structured programming five days a week. Teens in our Intensive Outpatient Program attend three hours of programming three/four days a week. Our programs serve as a smooth transition for teens stepping down from an inpatient or residential treatment program, or as a higher level of treatment for teens requiring more structure than just weekly outpatient therapy. In both telehealth programs, teens receive individual and family sessions, multiple group sessions a day, psychiatric evaluations and educational/academic support (all via HIPAA-compliant video conference). Teens also participate in experiential activities like music therapy, art therapy, meditation, and yoga. Our program setting is ideal for those who have already completed a residential, inpatient, or partial hospitalization treatment program and are looking for further care. Some individuals may qualify for the outpatient program without first having the need for a higher level of care. San Jose Behavioral Health offers an intensive outpatient program (IOP) for adults age 18 and older who are in need of treatment for mental health disorders. In this program, patients can participate in flexible, person-centered care while also carrying out their daily responsibilities outside of treatment. At our outpatient treatment center in Santa Cruz, California, we strive to provide a welcoming and comfortable environment where patients can begin their healing journeys. Adults may enter this program directly or as a step-down to a less restrictive level of care. Typically, people remain in our IOP for eight to 12 weeks, and they attend treatment three hours a day, three days a week for the duration of their programming. Common modalities we utilize at this level of care include cognitive behavioral therapy, dialectical behavior therapy, eye movement desensitization and reprocessing, motivational interviewing, mindfulness, and somatic therapy.

At Evolve, we provide the highest caliber of evidence-based residential treatment for adolescents 12-17 struggling with depression, anxiety, substance abuse, self-harming or high-risk behaviors, suicidality, aggression, defiance, trauma, and other emotional/behavioral issues. Our primary modes of treatment are Dialectical Behavior Therapy (DBT) and Cognitive Behavioral Therapy. At Evolve Lafayette, we believe treatment works best when it is individually tailored. Our clinical staff conducts a comprehensive evaluation of each teen pre- and post-admission in order to develop a customized, highly detailed treatment plan that is constantly updated based on treatment progress. Our robust clinical program includes individual and family therapy at least 5 times a week; weekly psychiatry; daily groups (on DBT Skills Training, Anger Management, Relapse Prevention, Life Skills, Healthy Relationships, and Seeking Safety); and daily academic support. Families from out-of-state participate in family therapy remotely. Teens also participate in experiential therapies (e.g. equine, music, art, fitness, yoga, and more) as well as SMART Recovery programs for those struggling with dual diagnoses.
