Hazelden Betty Ford's Center for Teens, Young Adults and Families in Plymouth, MN is where we've been helping those impacted by the disease of addiction for more than 30 years. Since 2002, MHS has provided intensive Dialectical Behavior Therapy services for individuals with significant difficulties in daily functioning. Many of our clients struggle with severe depression, anxiety, post-traumatic stress, substance abuse, relationship problems and safety issues that require ongoing monitoring and treatment. Our programs can be an alternative to frequent hospitalizations by providing skills training and support designed to manage emotions, thoughts, behaviors, and crises while remaining safe in the community. PrairieCare provides each individual patient the psychiatric care truly needed with specialized psychiatric services for all ages. Our comprehensive care includes Inpatient Hospitalization focused on safety and stabilization, to address acute health and safety concerns; Residential Services for youth ages 10 to 18, with an average length of stay between two and four months; Partial Hospitalization Program (PHP) offers intensive crisis stabilization, specialized psychiatric assessment, and intensive therapeutic services; Intensive Outpatient Programs (IOP) offering programming four or five days per week, with psychoeducation and group sessions; and Clinic/Outpatient Clinic Services providing specialized psychotherapy, psychiatry, medication management, and evaluation. Welcome to Hazelden Betty Ford's outpatient alcohol and drug treatment center in Maple Grove, Minnesota where hope is real - and help is ready. Our expert team focuses on the physical, spiritual and mental aspects of addiction in order to treat the whole person. At our Maple Grove clinic we specialize in evidence-based care for adults as well as treatment for co-occurring mental health conditions.

Located in the heart of the Embarcadero district, Foundations San Francisco offers a comprehensive intensive outpatient program and Partial Hospitalization for addiction and mental health conditions in a convenient urban setting. We provide highly individualized, confidential treatment to help you in your recovery. Our masters-level therapists offer Dialectical Behavior Therapy (DBT) and Cognitive Behavioral Therapy (CBT) and other evidence-based therapies alongside mindfulness and holistic practices with a flexible schedule. In addition, we offer 12-Step-optional programs where clinical decisions are driven by your unique needs. Participants attend the program for approximately 25 days or around 5 to 8 weeks. Participants attend the program a minimum of 3 days a week up to 5 days a week depending on the level of support needed. The Day IOP runs Monday through Friday 3 hours a day, 12PM –3PM and the Evening IOP runs Monday through Thursday 5PM – 8PM. OP is now available including 1 group and 1 individualize session per week.
